Statute 65-507: Records of maternity centers and child care
facilities; confidentiality.
(a) Each maternity center
licensee
shall keep a record upon forms
prescribed and provided by the secretary of health and environment and the
secretary of social and rehabilitation services which shall include the name of
every patient, together with the patient's
place of residence during
the year preceding admission to the center and the
name and address
of the attending physician. Each
child care facility licensee shall keep a record
upon forms prescribed and provided by the secretary of health and
environment which shall include the name and age of each
child received and cared
for in the facility; the name of the physician who attended
any sick children in the facility, together with the names and addresses of the
parents or guardians
of such children; and such other information as the secretary of health
and environment or secretary of social and rehabilitation services may require. Each
maternity center licensee and each child care facility licensee
shall
apply to and shall receive without charge from the
secretary of health and
environment and the secretary of social and rehabilitation services forms
for such records as may be required, which forms shall contain a copy of this
act.
(b) Information obtained under this section shall be confidential and
shall not be made public in a manner which would identify individuals.
